Patient Paperwork


Please download the appropriate paperwork for your upcoming visit with Dr. McBee, fill out all forms completely, sign and date them, and remember to bring them on the date of your appointment. If your apointment is for psychotherapy, you will need to download the New Patient Paperwork for Psychotherapy; if you are being seen for a one-time psychological evaluation (such as those for bariatric and spinal cord stimulator surgeries), please download from the link that says Patient Paperwork for Psychological Evaluations.

To download the file, click the right button of your mouse, choose "save link as..., and then save the PDF file to your computer in a location that you can easily remember. Then print the packet in its entirety and complete and sign all sections.

Please Note:
  1. Both packets have a release for your insurance company, the packet for the psychological evaluations has a second release for your surgeon or referring physician.

  2. Please be aware that the "Credit Card Guaranty of Payment" agreement is necessary for all patients to complete - it does not mean that you must or even should pay with a credit card for any money owed on your visit(s), however it is only an agreement for your card to be charged in the event there is ever an overdue balance once the office has billed you and given you final notice that you have a balance that is unpaid. Because it would not be ethical or legal for us to require it only of certain patients, all patients must complete this form (even those who are paying cash or whose insurance covers 100% of the services). Please bring the card you use for this form with you for your 1st appoIntment. Thank you for your understanding!
